Associate Clinical Nurse Specialist

An exciting opportunity has arisen for a nurse with special interest in palliative and end of life care to join the Earl Mountbatten Hospital Palliative Care Team based at St Mary’s, as an Associate Palliative Nurse Specialist.

If you are passionate about providing the very best palliative and end of life care to your patients we have an exciting opportunity for you to join our Hospice.

We are seeking to recruit a highly motivated and enthusiastic nurse with a genuine interest in palliative and end of life care, along with a commitment to continuous learning, team work and mutual support.

The post holder will work directly in clinical areas and alongside the ward based teams to support the palliative and end of life care needs of patients including symptom management; psychological care; rehabilitation and preferred place of care.

The successful candidate will need to demonstrate clinical expertise within the field of palliative and end of life care as well as excellent communication skills and experience of teaching in a clinical setting.

You will be required to work flexibly which will include shift work in the clinical setting, inclusive of weekends and nights.

Staff Nurses

Working across Inpatient Unit and Community Teams

Salary £22,128 to £28,746 (AfC Band 5)
37.5 hours per week worked on a shift rotation including nights
 

We are recruiting NMC Registered Nurses to join us who have experience of providing palliative care in a Hospital, Hospice or Community settings.

You will be based in our community and hospice inpatient units, delivering individualised care to our patients and supporting their families to enable them to be cared for in their preferred place of care.

You will be required to work rotating shifts, which will be predominately be based around nights, although you will be required to cover weekends, bank holidays and some day shifts.

You will be responsible for liaising and supporting the wider health and social care workforce in the delivery of exceptional end of life care across the Island.

You will have at least one year’s post registration experience and ideally hold a post qualifying qualification relating to palliative and end of life care. In addition, you will have demonstrable commitment to continuous learning, team work, and mutual support.

ADMIN SUPPORT OFFICER

Care Coordination Centre

 20 hours per week - 3 permanent contracts

(you will be required to provide additional cover)
 

Salary £16,655.40 per annum pro-rata plus benefits

Exciting opportunities are available for experienced administrators to join our new coordination centre which will be launched in January 2018.

The Hospice will be providing 24 hour Advice and Support Line for patients, families, carers and professionals.  Our services will help to make a difference to people across the Island who have life limiting illnesses or who may be approaching end of life.  Our services aim to ensure that patients' wishes are respected and that support is in place to help them, their families and the professionals caring for them.  Our aim is to help support patients to remain in their preferred place of care, which is increasingly the patient's home.

As Admin Support Officer you will take the lead in answering calls into our coordination centre, supporting the triaging of referrals into the hospice, and associated services.  We are looking for a friendly, dynamic and flexible individual who will provide general administration support, including the processing of referrals, inputting data into our electronic patient records, answering incoming calls and preparing meeting agendas and minutes. 

You will be an excellent communicator with good interpersonal and customer care skills to enable you to deal with conversations in a professional and friendly manner, as you will have interaction with patients and families during sensitive times.   You must be competent in the use of Microsoft Office including Word, Excel and Outlook. You will have the ability to use your own initiative and enjoy learning new skills, be self-motivated, have good interpersonal skills.
