- The Hospice@Home team helps facilitate an increase in overall provision of specialist, community palliative nursing care to enable patients to be cared for at home during the final days of their lives.
- We are a small team of nurses, with palliative and community skills, who aim to support the GPs, District, Marie Curie and Macmillan nurses in providing 24 hour access to care for terminally ill patients in their own homes.
- We endeavour to alleviate symptoms and anxieties to allow patients to end their lives peacefully and with dignity in the place of their choice.
- We also aim to maximise the overall quality of the patients' and their carers' lives by acknowledging their physical, psychological, social and spiritual needs in order to restore a sense of worth, purpose and hope.
- We aim to offer the highest quality palliative care nursing to our patients and support for their families during the last few days of life.
- A 'Just in Case' box can be provided to ensure necesary prescribed drugs are available out of normal pharmacy hours.
